Use this service to claim for an annual health and welfare review with your vet.
If you have multiple businesses that are registered to your rural payments account, you'll be asked which one you're claiming for.

You’ll need:
- your Rural Payments service customer reference number (CRN)
- your Rural Payments service password
- the date when the vet last visited the farm to carry out the review
- the date the vet completed endemic disease or condition testing
- the number of beef cattle, sheep and pigs the vet tested - you do not need to provide the number of dairy cattle tested
- the name of the vet
- the vet’s Royal College of Veterinary Surgeons (RCVS) number
- the laboratory unique reference number (URN) for the test results
